Mandatory Co-Parenting Programme (CPP) Consultation - For Myself


MSF’s CPP Consent and Acknowledgement

  1. I declare that the information provided in this application is true to the best of my knowledge, information and belief.
  2. I understand and agree that these phrases used in this application form have the following definitions:
    1. “Personal Information”includes my:
      1. personal data (e.g. name, NRIC number, email address, gender, family/household structure);
      2. financial data (e.g. income); and
      3. other information (e.g. employment, marriage) provided by me/us in my/our registration for this Programme.
    2. “Singapore Public Agencies”includes, (a) the Government, including any ministry, department, agency, or organ of State, (b) any tribunal appointed under any written law, (c) any statutory body, but excludes a Town Council established under section 4 of the Town Councils Act (Cap. 329A).
    3. “Participating Agencies”refers to organisations which are involved in the provision of the Programmes and have been approved by the Government of Singapore to collect, share or use Personal Information under a valid consent form. A list of Participating Agencies and Programmes can be found at this link.
    4. “Programme(s)”refers to Mandatory Co-Parenting Programme (CPP).
  3. I understand that I may be required to furnish additional Personal Information during the Programme(s).
  4. I allow the Singapore Public Agencies and Participating Agencies to collect, share and use my Personal Information for the following purposes:
    1. to verify my identity and relationship for the Programme(s) that I am applying for in this application;
    2. to determine my and my Family Member(s)’ and/or spouse’s eligibility for the Programme(s);
    3. to enrol me and my Family Member(s) and/or spouse in the Programme(s);
    4. to conduct the Programme(s);
    5. to deliver the services under the Programme(s) to me and my Family Member(s) and/or spouse; and
    6. for data analysis, evaluation and policy-making for the Programme(s).
  5. I understand that my Personal Information will not be shared with non-participating agencies and organisations.
  6. My consent remains valid until I withdraw it in writing. I accept that it will take up to 10 working days from the date of receipt before the withdrawal of consent takes place.
  7. This consent shall be governed by and construed in accordance with the laws of the Republic of Singapore.
